Girl Scout Membership Forms

To become a Girl Scout, each member must fill out a membership form. At this time membership forms can only be accepted in-person or by mail, there is no online registration. The girl form requires a parent or guardian signature. Each form must be accompanied by the $10 membership dues (Financial Aid is available).

Already in a troop or know which troop you will join?

  • Give the completed form to your Troop Leader

New to Girl Scouts, Juliette’s & Independent Scouts

  • Send the form to your Service Unit Registrar. If you do not know who that person is, please contact your Community Development Director.

Other Forms

  • The Annual Permission form that was in use in one of our legacy areas is now available to all troops and is approved as an option for obtaining permission for girls to participate in local trips. To use this form, leaders must agree to communicate the details regarding these trips to the parents and to carry the Annual Permission forms along with the girls' Health History forms at all times.

  • A new Health History form has been created with annually updated signature lines so that parents can review the information and if nothing has changed simply re-sign and date the form.

Family Partnership

When filling out your form, please also support our Family Partnership Campaign! Family Partnership is your family’s opportunity to support your daughter’s experience. Girl Scouts does not require mandatory program fees. 100% of family donations remain in our council to support our programs and operations.

All gift sizes are needed and appreciated.
$50 Can sponsor one hour of hands-on outdoor education programs for 25 girls.
$100 Can support one adult leader with free training and free resources.
$300 Can provide one girl with a year’s worth of program opportunities plus her leader’s training

How can I give to Family Partnership?

  1. Donate directly on the membership form.
  2. Give through your workplace campaign and reference your Troop Number & “Family Partnership.” Ask if your employer will match your gift!
